WholePath Lifestyle Medicine provides lifestyle-focused education, coaching, and support grounded in evidence-based lifestyle medicine principles. Services are non-emergency and are not intended to replace primary care or specialty medical care.
This practice does not provide urgent care, emergency services, or comprehensive medical management. Patients are encouraged to maintain an ongoing relationship with a primary care provider.

The initial visit serves as an evaluation to determine whether WholePath Lifestyle Medicine is an appropriate fit for a patient’s goals and needs.
Not all individuals, conditions, or goals are appropriate for this model of care. When appropriate, patients may be referred back to their primary care provider or to another healthcare professional better suited to address their needs.

WholePath Lifestyle Medicine is a telehealth-based practice. Communication outside of scheduled visits is limited to administrative or scheduling matters.
This practice does not provide medical advice via email, phone calls, or text messages. Messages are typically reviewed and answered within 1–2 business days.

Initial Visit: 60–75 minutes
Follow-Up Visits: Typically 30–45 minutes
Visit frequency is individualized based on patient goals, preferences, and progress. Many patients choose follow-up visits every few weeks or monthly for ongoing support and accountability.

Appointments are scheduled in advance and payment is collected at the time of booking.
If, after reviewing intake information prior to the appointment, the provider determines that the practice is not an appropriate fit, the appointment may be canceled and a refund issued.
If the determination is made during or after the visit, the visit fee is considered earned and is non-refundable, as professional time, assessment, and guidance have been provided.

Appointments may be canceled or rescheduled up to 24 hours before the scheduled visit time.
Cancellations made with less than 24 hours’ notice, or missed appointments, may not be eligible for a refund.
Refunds
Package visits are refundable only for unused visits. Any completed visits will be charged at the standard individual visit rate, and the remaining balance will be refunded accordingly.
Fit & Scope
WholePath Lifestyle Medicine focuses on lifestyle-based, preventive, and supportive care. If at any point it becomes clear that services are not the right fit, this will be discussed openly and remaining unused services will be handled according to the refund policy above.

All services are provided via secure telehealth platforms. By scheduling an appointment, you acknowledge and consent to receiving services via telehealth and understand the inherent limitations of remote care.
