I’m Ayé Diallo, a Physician Associate certified in Lifestyle Medicine by the American College of Lifestyle Medicine (ACLM), with a background in primary care, clinical research, and global health.
Over the course of my career, I’ve worked with patients managing chronic conditions, navigating complex health systems, and trying—often unsuccessfully—to make sense of conflicting health advice. What I saw repeatedly was a lack of time, continuity, and practical guidance around lifestyle factors that profoundly influence health.
My approach integrates clinical training with evidence-based lifestyle medicine principles, with an emphasis on patient education, prevention, and long-term health support- so you leave visits with clarity and a plan you can actually use.

Initial Lifestyle Medicine Visit
$250 | 60—75 minutes
This first visit is a comprehensive, dedicated consultation focused on understanding your health history, goals, and lifestyle patterns. Together, we identify priorities and develop a realistic, personalized plan aligned with your needs and values.

Follow-Up Visits
$150 | 30–45 minutes
Follow-up visits are designed to support progress, address barriers, and refine your plan over time with adjustments made as needed to support sustainable change. The frequency of visits is individualized. Many patients choose visits every few weeks or monthly, depending on their goals and preferences.

Packages (Optional)
For patients who prefer to plan ahead, optional visit packages are available. Packages are offered to support continuity of care and are never required. Details can be discussed during your visit.

Payment & Insurance
Payment is collected at the time of booking. Patients may use Health Savings Account (HSA) or Flexible Spending Account (FSA) funds when eligible and may submit receipts to their insurance provider for possible out-of-network reimbursement. Coverage and reimbursement vary by plan and insurance reimbursement is not guaranteed. Detailed receipts and superbills are available upon request.
